Present a memorandum to the ID Card Section. It should explain what happened to the card and when and must be signed.
Have it signed.
1.For Soldiers: memo must be signed by Soldier’s 1SG or commander with a statement that the Soldier has been counseled on safeguarding government property.2.2.For civilians: memo must be from Civilian Personnel Office.3.3.For contractors: memo must come from their trusted agent.4.4.For family members: memo must be signed by their active-duty or retired sponsor.5.
Memos can be obtained from ID Card Section customer service clerk. If sponsor is not available, then customer must have a proper power of attorney.
