Need to get a copy of your child’s immunization records in GA? Here’s how to do it
Parents, get your list ready for back-to-school shopping. You’ll need pencils, paper, a backpack and your child’s vaccination records.
Wait, that last part isn’t something you can get at the store.
So, how do you get it?
Well, here’s a step-by-step guide for you.
Get your documents ready
You will need:
The full name, date of birth and mother’s full name of the person whose immunization records you are requesting.
Valid, government-issued identification of the person requesting the immunization records. (Such as a state-issued photo driver’s license with address, a state-issued photo identification card with address or a U.S. passport or passport card with photo.)
Contact information, including a mailing address, email address and phone number.
Submit your request
You must fill out the online application, upload the necessary documents and submit the form. The immunization records request form is located online.
Next Steps
After you’ve submitted the form, please allow three to five business days for processing. After this:
You will receive an encrypted message with your complete immunization record from the Department of Public Health via email.
If you believe a vaccine dose is missing from your records, please contact the health care provider that administered the vaccine.
